Aesthetic dermatology: what is possible – and what makes sense

Aes­the­tic medi­ci­ne does not live from the num­ber of tre­at­ments, but from choo­sing the right one. A face never ages on a sin­gle level, and a sin­gle mea­su­re the­r­e­fo­re rare­ly pro­du­ces a coher­ent result. That is why every tre­at­ment beg­ins with ana­ly­sis – and an honest con­ver­sa­ti­on about what can rea­li­sti­cal­ly be achie­ved and what cannot.

The four levels of facial ageing

Visi­ble age­ing hap­pens on four levels at once. The skin loses col­la­gen, ela­s­ti­ci­ty and mois­tu­re. The fat com­part­ments of the face shrink and shift down­wards. The mus­cles etch lines into the skin through repea­ted expres­si­on. And the bone chan­ges its con­tour over the deca­des, so that sup­port­ing struc­tures are lost. Smoot­hing the sur­face when the actu­al cau­se is a loss of volu­me will always dis­ap­point. Assig­ning the fin­dings to the cor­rect level is the real cli­ni­cal skill – ever­y­thing after that is technique.

From consultation to treatment plan

At the first con­sul­ta­ti­on we look at your face at rest and in move­ment, ide­al­ly along­side your own pho­to­graphs from ear­lier years. From this we build a plan with prio­ri­ties: what will make the big­gest visi­ble dif­fe­rence, what is optio­nal, what needs time. We often spread tre­at­ments deli­bera­te­ly across seve­ral appoint­ments – it is easier on the bud­get, allows inte­rim reviews, and pre­vents seve­ral effects set­ting in at once so that none can be jud­ged on its own.

Natural results are a question of dosage

Unna­tu­ral results rare­ly come from the wrong pro­duct. They come from too much of it, or from trea­ting the wrong place. We the­r­e­fo­re pre­fer to work con­ser­va­tively and review after­wards: a result can always be built on, whe­re­as too much cos­ts time and some­ti­mes a cor­rec­tion. With hyalu­ro­nic acid the­re is a way back – excess mate­ri­al can be dis­sol­ved using hyalu­ro­ni­da­se.

Safety: why it matters who treats you

Injec­tions in the face are pla­ced clo­se to ves­sels and ner­ves. Ana­to­mic­al know­ledge, ste­ri­le con­di­ti­ons, pro­ducts appro­ved in Switz­er­land and pre­pared emer­gen­cy manage­ment are not details here, they are pre­re­qui­si­tes. How long results last

Aes­the­tic results are not a fixed sta­te but a cur­ve. Mus­cle-rela­xing tre­at­ments usual­ly last three to six months, fil­lers rough­ly six to eigh­te­en months depen­ding on the pro­duct and the area, while col­la­gen-sti­mu­la­ting pro­ce­du­res build up over weeks and last cor­re­spon­din­gly lon­ger. Kee­ping to the rhythm means less pro­duct per ses­si­on than start­ing again once ever­y­thing has ful­ly bro­ken down. Frequently asked questions

At what age do aes­the­tic tre­at­ments make sense?

The­re is no fixed age limit, only a sui­ta­ble moment for each con­cern. In your twen­ties and thir­ties it is usual­ly about skin qua­li­ty, the after­math of acne, or pig­men­ta­ti­on – that is, pre­ven­ti­on and rege­ne­ra­ti­on. From around for­ty, loss of volu­me and firm­ness are added. What mat­ters is the cli­ni­cal fin­ding, not the year of birth. With very young pati­ents we are deli­bera­te­ly cau­tious and will advi­se against tre­at­ment whe­re appropriate.

Will the result look natural?

That is a ques­ti­on of dosa­ge and pla­ce­ment, not of the pro­ce­du­re its­elf. Our aim is a res­ted, coher­ent face – not a dif­fe­rent one. We the­r­e­fo­re work step by step: a litt­le less to begin with, then a review after two to three weeks with the opti­on of adding pre­cis­e­ly whe­re it is needed.

How long do results last?

Mus­cle-rela­xing tre­at­ments gene­ral­ly last three to six months. Hyalu­ro­nic acid fil­lers last rough­ly six to eigh­te­en months depen­ding on the pro­duct, the area and your meta­bo­lism. Col­la­gen-sti­mu­la­ting pro­ce­du­res such as micro­need­ling, radio­fre­quen­cy or ultra­sound build their result over seve­ral weeks and last lon­ger, but usual­ly requi­re a cour­se of treatments.

How much down­ti­me should I plan for?

This depends very much on the pro­ce­du­re. After injec­ta­bles you are usual­ly pre­sen­ta­ble imme­dia­te­ly, alt­hough small swel­lings or brui­ses are pos­si­ble and can remain visi­ble for a few days. After micro­need­ling or peels the skin is red for one to three days. Befo­re an important occa­si­on, it is best to allow two weeks of buffer.

Can tre­at­ments be combined?

Yes, and many com­ple­ment each other – volu­me res­to­ra­ti­on and skin qua­li­ty, for exam­p­le. The sequence and the inter­vals are what mat­ter: some com­bi­na­ti­ons we car­ry out on the same day, others we deli­bera­te­ly sepa­ra­te by two to four weeks so that heal­ing pha­ses do not over­lap and each effect can be asses­sed on its own.

Does health insu­rance cover aes­the­tic treatments?

No. Tre­at­ments wit­hout a medi­cal indi­ca­ti­on are paid pri­va­te­ly in Switz­er­land. The situa­ti­on dif­fers whe­re the­re is an under­ly­ing con­di­ti­on – exces­si­ve swea­ting, seve­re acne or a trou­ble­so­me scar after an acci­dent, for exam­p­le. We dis­cuss open­ly at the first con­sul­ta­ti­on whe­ther an indi­ca­ti­on exists.

Who car­ri­es out the treatments?

Injec­ta­bles, laser tre­at­ments and ener­gy-based pro­ce­du­res are per­for­med by our doc­tors. Cos­me­tic tre­at­ments such as facials or super­fi­ci­al peels are car­ri­ed out by our cos­me­tic team, working under the cli­ni­cal gui­dance of the der­ma­to­lo­gy department.

How does this dif­fer from a clas­sic beau­ty treatment?

Aes­the­tic der­ma­to­lo­gy uses pro­ce­du­res that reach deeper lay­ers of the skin – injec­tions, lasers, radio­fre­quen­cy – and that requi­re a medi­cal indi­ca­ti­on. Cos­me­tic tre­at­ments work more super­fi­ci­al­ly, care for the skin and impro­ve its appearance, and lend them­sel­ves well to cour­ses of tre­at­ment.
