In gynecomastia the male mammary gland tissue enlarges. This must be distinguished from pseudogynecomastia, in which fatty tissue is mainly responsible for the enlargement. Treatment can differ according to the type of tissue and the extent of the enlargement. Liposuction, removal of mammary gland tissue or a combination of both procedures are possible options.
Gynecomastia or fatty tissue?
In true gynecomastia the mammary gland tissue is enlarged. A firmer structure behind the nipple is characteristic.
In pseudogynecomastia the cause is excess fatty tissue. This is typical in overweight or after weight gain.
In the majority of cases a mixed form is present. The examination beforehand is one of the most important steps.
Causes of male breast enlargement
The reasons for gynecomastia range from hormonal changes through medication and metabolic disorders to weight gain or genetic factors. Before any surgical treatment, a treatable cause must be ruled out.
Not infrequently no clear cause is found. Once the breast has enlarged, it very rarely recedes on its own.
When surgery makes sense
Temporary gynecomastia is rare but does occur, for example during puberty. However, if the breast enlargement persists or leads to psychological or physical strain, surgical correction is an option. A specialist examination always comes first.

If it is mainly fatty tissue, liposuction may be sufficient. The excess tissue is suctioned off through small access points and the breast contour is gently modelled.
In gynecomastia we additionally have to remove excess mammary gland tissue through a small incision at the edge of the areola. We often combine both procedures.
What about more pronounced findings?
If the breast is greatly enlarged, there is also excess skin. Removing fat or glandular tissue alone is then not sufficient. The excess skin has to be tightened so that the breast contour remains stable and looks natural. Various techniques are available to us at the Swiss Derma Clinic for this purpose.
Healing and compression
After the procedure a compression vest has to be worn. It supports the adaptation of the tissue and consolidates the new contour.
Mild to moderate swelling, bruising or a feeling of tightness are normal in the first weeks. The tissue first has to settle. The breast shape develops continuously over several weeks to months.
