Male breast reduction (gynecomastia) in Zurich

Almost one in three men over 50 has gynecomastia: an enlarged breast. Characteristic are bulges behind the nipple or an overall feminine-looking breast contour. Affected men avoid tight clothing or certain sporting activities. Not infrequently, psychological well-being and therefore quality of life are impaired.

The causes of breast enlargement in men are varied. What matters to us is whether it involves excess fatty tissue, enlarged mammary gland tissue or a combination of both. A specialist examination is necessary for this.

Your surgeon at the Swiss Derma Clinic is Dr. med. (I) Nunzioluca Chianese, specialist FMH for Plastic Surgery.

In brief

In gynecomastia the male mammary gland tissue enlarges. This must be distinguished from pseudogynecomastia, in which fatty tissue is mainly responsible for the enlargement. Treatment can differ according to the type of tissue and the extent of the enlargement. Liposuction, removal of mammary gland tissue or a combination of both procedures are possible options.

Gynecomastia or fatty tissue?

In true gynecomastia the mammary gland tissue is enlarged. A firmer structure behind the nipple is characteristic.

In pseudogynecomastia the cause is excess fatty tissue. This is typical in overweight or after weight gain.

In the majority of cases a mixed form is present. The examination beforehand is one of the most important steps.

Causes of male breast enlargement

The reasons for gynecomastia range from hormonal changes through medication and metabolic disorders to weight gain or genetic factors. Before any surgical treatment, a treatable cause must be ruled out.

Not infrequently no clear cause is found. Once the breast has enlarged, it very rarely recedes on its own.

When surgery makes sense

Temporary gynecomastia is rare but does occur, for example during puberty. However, if the breast enlargement persists or leads to psychological or physical strain, surgical correction is an option. A specialist examination always comes first.

Your treatment at the Swiss Derma Clinic

If it is mainly fatty tissue, liposuction may be sufficient. The excess tissue is suctioned off through small access points and the breast contour is gently modelled.

In gynecomastia we additionally have to remove excess mammary gland tissue through a small incision at the edge of the areola. We often combine both procedures.

What about more pronounced findings?

If the breast is greatly enlarged, there is also excess skin. Removing fat or glandular tissue alone is then not sufficient. The excess skin has to be tightened so that the breast contour remains stable and looks natural. Various techniques are available to us at the Swiss Derma Clinic for this purpose.

Healing and compression

After the procedure a compression vest has to be worn. It supports the adaptation of the tissue and consolidates the new contour.

Mild to moderate swelling, bruising or a feeling of tightness are normal in the first weeks. The tissue first has to settle. The breast shape develops continuously over several weeks to months.

FAQ

Männliche Brustverkleinerung (Gynäkomastie) FAQ

Can gynecomastia recede through sport or weight loss?

If the enlarged breast consists mainly of fatty tissue, losing weight can improve the contour. If mainly mammary gland tissue is present, this does not usually change through training or diets. This is why the distinction between gynecomastia and pseudogynecomastia is important.

How do I recognise whether it is fatty tissue or true gynecomastia?

True gynecomastia presents as rather firm tissue. Fatty tissue, by contrast, usually feels softer and is distributed more evenly. As mixed forms are common, the cause can only be assessed during a medical examination.

Are visible scars left after a male breast reduction?

The access points for liposuction are small. If we additionally remove mammary gland tissue, we need an incision at the edge of the areola. The scars fade during the course of healing.

When can I do sport again after the operation?

Light physical activity is possible after a few days. You should refrain from strength training, intensive sport and strain on the chest muscles for several weeks. A compression vest supports healing and the adaptation of the tissue. We discuss the details with you individually as part of your treatment at the Swiss Derma Clinic in Zurich.

Can gynecomastia occur again after the operation?

The removed mammary gland tissue does not grow back. Significant weight gain, hormonal changes or certain medications can still influence the appearance of the chest. Clarifying possible causes is part of the treatment and is essential before every procedure.

How long does it take for the final result to show?

After the procedure, swelling typical of surgery and temporary hardening are normal. A large part of the swelling subsides in the first weeks. It takes several months until the breast contour has completely stabilised.

Does health insurance cover the costs of a male breast reduction?

In certain cases cost coverage may be possible. The requirements should be clarified individually in advance.
